    in 1992. According to INSAG-1, the main cause of the accident was the operators' actions, but according to INSAG-7, the main cause was the reactor's design. Both reports identified an inadequate "safety culture" (INSAG-1 coined the term) at all managerial and operational levels as a major underlying factor. Wikipedia summery of the accident report. That being said the test they were doing fell off the rails pretty quickly, and should not have been tried in the first place.

    @vixsage7 The control rods got stuck on the way down due to poor maintince, design, and operator error by not responding in time leaving only the graphite tips in the pool. Idealy what should have happened is lowering the control rods early leaving room for the small surge to safly happen untill the rods where fully inserted halting the reaction.

    shortened: the reactor blew out due to the control rod design; the tips of the rods were made of graphite which accelerated the reaction instead of slowing it down. As AZ-5 was pushed, all the rods went in and this meant the graphite tips entered the reaction first, speeding it up and causing an uncontrollable dangerous power surge

    In short it was the standerd "horrible accident no on could see coming" The "no one" in most cases are managment, goverment, and money pinchers. Think about that tidbit the next time you hear the classic "who could have seen that comming" after a horrific tragity.

    Fun Fact the meltdown was caused by extremly poor maintince, a crew of inexpirinced operators, and poor reactor design. The goverment wanted to do the surge test during the day but the expirinced operators refused because they knew it would go out of control, do to the 40% functional state of the control system, so thats why the trainees were made to do it becuase they could be convinced it was safe.
